过敏性紫癜急性期患儿免疫功能研究

摘    要:目的观察急性期过敏性紫癜(HSP)患儿的细胞和体液免疫变化,以探讨其发病机制。方法分别采用流式细胞仪(FCM)和免疫散射比浊法检测急性期HSP20例患儿外周血淋巴细胞亚群和血清Ig、补体含量,10例健康儿童为对照组。结果1.HSP组CD3 、CD4 细胞和CD4 /CD8 低于对照组(P<0.001,P<0.001,P<0.05);CD19 细胞高于对照组(P<0.05);CD8 细胞和CD16 /CD56 细胞与对照组比较无显著性差异(P>0.05);2.HSP组IgA、M和C3高于对照组(P<0.001,P<0.05,P<0.05),IgG低于对照组(P<0.001);两组C4无显著性差异(P>0.05)。结论HSP急性期患儿细胞免疫功能低下,体液免疫功能亢进。补体激活增加,提示免疫功能改变在HSP发病机制中起重要作用。

Abstract:Objective To investigate immunological function in acute phase of Henoch-Schonlein purpura(HSP) in children,in order to explore the role of immunological injury in pathogenesis of HSP.Methods Twenty children with HSP in acute phase were enrolled in this study,ten healthy children were control group.The antigen markers of T-lymphocyte,B-lymphocyte and NK cells , including CD3,CD4,CD8,CD_ 19 ,and CD_ 16/56 expression on the surface of peripheral blood lymphocytes were analyzed by flow cytometry,the serum levels of IgA,IgG,IgM and the C_3,C_4 were detected by immunol nephelometer.Results 1.CD3 and CD4 cell,ratio of CD4 /CD8 cell in HSP group were significantly lower than those in control group( P <0.001, P <0.01, P <0.05),CD_ 19 cell was significantly higher than that in control group( P <0.05),CD8 cell and CD_ 16 /CD_ 56 cell were not significantly difference between HSP and control group( P >0.05). 2.The serum concentration of IgA,IgM and C_3 in HSP group were significantly higher than those in control group( P <0.001, P <0.05, P <0.05).The level of IgG in HSP group was significantly lower than that in control group( P <0.001),the level of C_4 was no significantly difference between two groups( P >0.05).Conclusion Our study showed that there are abnormal changes in humoral immunity and cell mediated immunity in acute phase of HSP,the decrease of T-lymphocyte and increase of B-lymphocyte may be involved in the pathogenesis of HSP.
