半规管阻塞技术治疗迷路瘘管初步报告

目的探讨临床应用半规管阻塞技术治疗迷路瘘管的安全性与有效性。方法分析12例慢性化脓性中耳炎(胆脂瘤型)并发迷路瘘管患者的资料,均为水平半规管瘘管,瘘管直径大于2mm,骨性半规管破坏,其中Ⅱ型10例,Ⅲ型2例。术中清理胆脂瘤基质和瘘管周围的炎性病变组织后,用颞肌填塞瘘管两端骨性管腔,表面再以骨粉生物胶混合物覆盖。结果术后随访至少1年,所有患者消除了眩晕,术后气骨导听力无明显变化。结论半规管阻塞技术治疗迷路瘘管能有效消除眩晕而保存听力。

Primary clinical application of semicircular canal occlusion for treatment of labyrinthine fistula

Objective To discuss the safety and efficacy of applying semicircular canal occlusion for treatment of labyrinthine fistula.Methods Twelve patients with cholesteatomatous chronic suppurative otitis media combined with labyrinthine fistula were retrospectively reviewed. All the patients had lateral semicircular canal fistula with a diameter larger than 2 mm. After removal of cholesteatoma matrix and inflammatory tissue around fistula, the bony semicircular canal lumen were packed with temporalis, then overlaid with the mixture of the bone chips and biological glue. Results The patients were followed up for at least one year. The vertigo was eliminated in all the patients and the hearing thresholds were not significantly changed postoperatively. Conclusion Semicircular canal occlusion could eliminate vertigo in patients with labyrinthine fistula,meanwhile had no effect on cochlear function.
