保留尿道的前列腺摘除术121例报告

目的：总结保留尿道的前列腺摘除术的经验教训，以便更好地应用于临床。方法：应用保留尿道的前列腺摘除术治疗前列腺增生患者121例，靠近膀胱颈部横切口显露尿道，必要时联合膀胱切开行增生腺体切除或者同时处理膀胱内病变。结果：121例患者手术均能够顺利完成，108例完整保留尿道，10例尿道撕裂经Ⅰ期修补后愈合，3例因尿道损伤严重无法修复，遗留不同程度后遗症。经后期治疗恢复。手术历时50～160min，平均90min。术中出血120～800ml，平均200ml。平均随访3—24个月，排尿满意。结论：本术式操作简单，术中出血少，并发症少，可以提高前列腺开放手术的治疗效果，减少术后并发症的发生，值得在基层医院推广。

The study of 121 cases patients treated with rostatectomy reserves rostatic urethra

Objective： To sumu the exerience of rostatectomy which reserves rostatic urethra, in order to aly in clinical theray more effectively. Methods： Rostatectomy was erformed on 121 cases of benign hyerlasia of rostate, horizontal incision near bladder neck was alied to show urethra clearly, associated with cystotomy or treat with diseased region in bladder at the same time. Results： All the 121 oerations were successful. 108 cases comlete reserved urethra, 10 laceration of urethra cases was healed after urethra neolasty at one time.3 cases could not be reaired because of severe urethra injury, followed by to some extent but all this was healed after treatment. Oeration time 50-160 min,average 90 min, bleeding in oeration： 120-800 ml, average 200 ml. In a eriod （3-24 months）of following u,the effects were satisfied. Conclusion： This kind of oeration is not comlicated, less bleeding and comlications, can improve the oeration effects of BPH, is worth to be oularized in rural hospital.
