胃癌中p27 cyclin E蛋白的表达及临床意义

目的　探讨 p2 7、cyclin E蛋白在胃癌中的表达及其与临床病理参数之间的关系。方法　用免疫组织化学 ( SP)法对 10例非瘤病变的正常胃黏膜、40例胃癌和癌旁黏膜进行染色 ,分析 p2 7、cyclin E蛋白表达及其与临床病理参数之间的关系。结果　 p2 7、cyclin E蛋白在正常胃黏膜与瘤旁黏膜表达差异无统计学意义 ( P>0 .0 5 ) ;p2 7在胃癌中的蛋白表达低于瘤旁黏膜和正常胃黏膜 ( P<0 .0 1) ,cyclin E在胃癌中的蛋白表达高于瘤旁黏膜和正常胃黏膜 ,且有过表达现象 ( P<0 .0 1)。肿瘤组织中 p2 7、cyclin E蛋白表达间无相关性 ( P=0 .0 64 ) ,但与 0 .0 5很接近 ,无统计学意义可能与样本数量少有关。肿瘤组织中 p2 7的阳性率为 2 8% ( 11/ 40 ) ,且在组织分化程度高、预后好者中阳性表达高 ( P<0 .0 5 ) ;cyclin E的阳性率为 75 % ( 3 0 / 40 ) ,中～强阳性约占半数 ( 14/ 3 0 ) ,且在胃癌浸润深、组织分化程度低者中表达高 ( P<0 .0 5 )。结论　 p2 7、cyclin E蛋白的异常表达与胃癌的发生、发展有关 ,并可能作为胃癌预后的指标

The expression and clinical role of p27 cyclin E protein in gastric carcinoma

Objective p27,cyclin E are prime cell cycle regulators and may be involved in tumorigenesis.p27,a cyclin-dependent kinase inhibitor,regulates progression from G 1 to S phase.Cyclin E is a late G 1 cyclin and its abnormalities have been reported in several cancers.The authors investigated their protein expression in normal gastric epithelium,tumor-sideward epithelium and carcinomatous tissues,and the correlation of their protein expression with the clinicopathologic features of gastric carcinoma.Methods The protein expressions of p27,cyclin E in 10 specimens of gastric noncarcinoma and 40 specimens of gastric carcinoma were studied by means of immunohistochemistry.Results There was no statistical significance between the expression′s differences of p27,cyclin E in the normal gastric epithelium and the tumor-sideward epithelium(P>0.05).On statistical principle,their data were consolidated,compared with the carcinomatous tissues group.The positive expressions of p27 in the carcinomatous tissues were significantly lower than those in the normal gastric epithelium and the tumor-sideward epithelium (P<0.01);by contraries,those of cyclin E were overexpressive,and significantly higher (P<0.01).In the carcinomatous tissues,there was no relativity among the protein expressions of p27 and cyclin E (P>0.05);but P=0.064 between p27 and cyclin E was close to 0.05,that might be correlative to the numbers of the specimens.At the same time,the p27 positive rate was 28% (11/40),which was frequently observed in high histopathological grade (P<0.05),and prognosis bona (P<0.05);the cyclin E positive rate was 75% (30/40),14 of which were medium-strongly positive,and often observed in deeply invasive (P<0.05),and poor histopathological grade (P<0.05).Conclusion Abnormal expression of p27,cyclin E may be related to the genesis and development of gastric carcinoma,and be used as a prognostic factor.
