交锁髓内钉治疗肱骨干骨折的疗效分析

目的：探讨交锁髓内钉治疗肱骨干骨折的临床疗效以提高肱骨干骨折的治疗水平。方法：总结2005年1月-2008年12月我院对21例肱骨干骨折进行交锁髓内钉固定治疗及预后的回顾性分析。从手术时间、愈合率、愈合时间等疗效指标和桡神经损伤、骨不连、肩肘关节功能障碍等并发症发生情况,这几个方面对手术疗效进行评价。结果：髓内钉固定强度大可早期负重,功能恢复好。骨折无延迟愈合及不愈合,无畸形愈合。顺行钉术后肩关节评估,根据Neer评分标准,优（90-100分）为13例（61.9%）,良（80-89分）为7例（33.3%）,可（70-79分）为1例（4.8%）〈70分为差0例。肘关节活动基本正常,无不良影响。结论：肱骨交锁髓内钉对于肱骨干骨折是一种固定可靠,并发症少,骨愈合率高,功能恢复良好的手术方法。

Efficacy analysis on treatment of humeral shaft fracture by interlocking intramedullary nail

Objective To approach the clinical efficacy of the treatment of humeral shaft fracture by interlocking intramedullary nail.Method The experiences in the treatment of 21 cases of humeral shaft fracture by interlocking intramedullary nail in our hospital were summarized and retrospective analysis of prognosis was made.The effects of surgery,such as the operation time,the healing rate,the healing time,radial nerve injury,bone non-union,and shoulder elbow dysfunction were evaluated.Results Intramedullary nail with enough strong fixation could load early and with good recovery function.There was no delayed union,nonunion and malunion or infection in all the cases.Shoulder function was assessed by Neer′s score,13 cases showed excellent results(61.9%),7 showed good(33.3%),1 showed fair(4.8%),none showed poor results.The activities of elbow joint was normal without adverse effects.Conclusion Interlocking intramedullary nail is a reliable method for humeral shaft fracture.The methods have fewer complications,higher bone healing rate and better functional recovery.
