腹腔镜胆囊切除术后上腹痛原因分析

目的:探讨腹腔镜胆囊切除术后上腹痛的原因。方法:回顾分析22例腹腔镜胆囊切除术后上腹痛患者的临床资料。结果:糜烂性胃炎3例,胆汁反流性胃食管炎1例,胆汁反流性胃炎伴胃粘膜散在出血点2例,胃窦部及十二指肠小溃疡各1例,胃癌1例,胰头癌1例,胆总管下段小结石2例,肝下间隙积液6例(1例为胆囊床迷走胆管漏,1例为右肝管电灼伤引起的胆漏),肋弓部损伤性疼痛4例。结论:腹腔镜胆囊切除术后出现的上腹痛应予以高度重视,只要具备高度责任心,术后疼痛是可以避免的。

The analysis of reasons of upper abdominal pain after laparoscopic cholecystectomy

Objective:To investigate the causes of upper abdominal pain after laparoscopic cholecystectomy(LC).Methods:The clinical data of 22 patients who had upper abdominal pain after LC were retrospectively analyzed.Results:In the 22 cases,there were 3 cases of erosive gastritis,1 case of bile reflux gastroesophagitis,2 cases of bile reflux gastritis with mucosa hemorrhage,1 case of sinus ventriculi ulcer,1 case of duodenal ulcer,1 case of gastric carcinoma,1 case of cancer of the head of the pancreas,2 cases of microlith in the inferior segment of the common bile duct,6 cases of hydrops in the inferior hepatic space,including 1 case of gallbladder bed aberrant bile duct leakage and 1 case of bile leakage caused by cauterization of right hepatic duct,and 4 cases of rib arch injury.Conclusions:More attention should be paid to the upper abdominal pain after LC.As long as surgeons have great responsibility,the postoperative pain could be avoided.
