p16、CD44v6和nm23-H1基因在胃癌中的表达及预后价值

[目的]研究p16、CD44v6和nm23-H1在胃癌中的表达规律,综合评价它们对患者预后的意义.[方法]用免疫组织化学LSAB方法,检测p16、CD44v6和nm23-H1蛋白在65例胃癌手术切除标本中的表达情况,并对它们与患者生存率的关系进行单因素和多因素分析.[结果]p16和nm23-H1的表达与胃癌的浸润、淋巴结转移和TNM分期呈负相关,CD44v6的阳性表达率与浸润深度、淋巴结转移和TNM分期呈正相关.p16和nm23-H1呈正相关,和CD44v6呈负相关.单因素分析表明,p16、CD44v6和nm23-H1是影响患者3年生存率的因素,多因素分析表明,只有p16是影响患者生存率的独立因素.[结论]p16、CD44v6和nm23-H1参与了胃癌的发展过程,对胃癌的预后有重要意义,p16可能是更有价值的预后指标.

Expressions of p16,CD44v6 and nm23-H1 Genes and Their Prognostic Value in Gastric Cancer

To investigate the expressions of p16, CD44v6 and nm23-H1 in patients with gastric cancer and their prognostic significance. p16, CD44v6 and nm23-H1 expressions were studied in 65 cases with gastric cancer by LSAB immunohistochemical technique. Their relationship with survival were evaluated by univariate and multivariate analysis. Expression of p16 and nm23-H1 were negatively corelated to depth of invasion, lymph node metastasis and TNM stage. Expression of CD44v6 was positively corelated to depth of invasion and lymph node metastasis and TNM stage. p16 was positively corelated to nm23-H1, and negatively corelated to CD44v6.Univariate analysis showed that the prognostic factors which influenced 3-year overall survival rate were p16, CD44v6 and nm23-H1. Multivariate analysis demonstrated that p16 was the only independent prognostic factor determining 3-year survival. [Conclusion] p16, CD44v6 and nm23-H1 may play some roles in the development of gastric cancer. Although p16, CD44v6 and nm23-H1 are of prognostic significance, p16 is the most valuble in evaluating prognosis for gastric cancer.
