温针经筋刺法治疗卒中痉挛性肩痛疗效观察

目的:温针经筋刺法治疗卒中后痉痉挛性肩痛的临床疗效和安全性。方法:共纳入60例脑卒中后痉挛性肩痛患者,温针经筋刺组(治疗组)和常规针刺组(对照组)各30例。治疗组采用温针灸结合经筋刺法配合康复治疗,对照组采用常规针刺配合康复治疗,两组在治疗前后进行VAS评分、肩关节活动度测量(ROM)、Barthel指数评分、改良的Ashworth量表评定。结果:两组在疼痛VAS评分、肩关节活动度、Ashworth分级等方面的评价结果比较,差异均有统计学意义(P<0.01),温针经筋刺组优于常规针刺组。结论:温针经筋刺法可以有效缓解痉挛性肩痛,缓解肌肉痉挛,增加肩关节活动角度,提高日常生活能力,疗效优于常规针刺方法,且安全无副反应。

Effective Observation on Muscle and Tendon Therapy of Acupuncture with Warmed Needle to Treat Post-stroke Spastic Shoulder Pain

Objective：This study was designed to evaluate the clinical effectiveness and safety of muscle and tendon therapy of acupuncture with warmed needle to treat post-stroke spastic shoulder pain.Methods： In the prospective single blind clinical controlled study,60 patients with post-stroke shoulder pain were randomly divided into two groups,and there were 30 neonates in each group.The test group apply muscle and tendon therapy of acupuncture with warmed needle,and the control group were given routine acupuncture therapy.Both group apply the conventional rehabilitation intervention and medicine for stroke prevention.Measuring the range of shoulder motion,ashworth and barthel at different intervals before and after treatment.And evaluating VAS every day during the treatment.Results： The results showed that both groups improved after treatment.The VAS,range of shoulder motion and ashworth score of the test group were more significant than that of the control group（P0.01）.During the treatment,no adverse reaction was occurred in all patients.Conclusion： Muscle and tendon therapy of acupuncture with warmed needle is a safe and effective method for treating post-stroke spastic shoulder pain.
