自身抗体对副肿瘤综合征诊断价值的探讨

目的　探讨自身抗体对副肿瘤综合征的诊断价值。方法　利用间接免疫荧光法对 48例疑诊副肿瘤综合征患者进行抗 Yo抗体、抗 Hu抗体、抗 Ri抗体测定和临床随访 ,并与正常人和神经系统其他疾病患者对照。结果　 48例中 ,1例病前有肺癌史 ,2例检查中发现肿瘤 ,4例随访 3～ 1 8个月后发现肿瘤。正常对照组和神经系统其他疾病组自身抗体均为阴性 ,患者组中有 1例呈副肿瘤性脑脊髓炎 ,其抗 Hu抗体阳性 ,相对分子质量为3 80 0 0 ,病理检查证实为小细胞肺癌。结论　自身抗体测定对此病的早期诊断有一定价值 ,但阳性率不高 ,其临床价值尚需进一步随访证实

The Role of the Autoantibodies in Diagnosis of Neurological Paraneoplastic Syndrome

Objective To investigate the role of autoantibodies in neurological paraneoplastic syndrome(NPS). Methods Using immunofluorescence method,we measured the anti Yo, anti Hu, and anti Ri anti bodies in the serum of 48 cases with suspected NPS, the patients were followedupandcomparedwithother neurological disorders and healthy controls. Results Of the 48 cases, one suffered from lung carcinoma before admission, then carcinoma were detected in two patients during examination and in 4 patients during follow up period (3 18 months). The autoantibodies were negative in healthy subjects and other neurological diseases, Anti Hu antibody, which molecular weight were 38 kd, was detected inapatientwith paraneoplastic encephalomyelitis, the histological diagnosis was then small cell lung carcinoma. Conclusions The study indicated that the present autoantibodies may be helpful in diagnosis of NPS, concerning abnormal percentage is not high, its role will be recognised with further study.
