婴幼儿肺炎血浆内皮素-1与肺动脉高压的关系

摘    要:探讨婴幼儿肺炎患儿血浆免疫活性内皮素-1(irET-1)水平与肺动脉高压之间的关系。方法对17例对照组、38例轻型肺炎及10例重型肺炎极期和恢复期测定血浆irET-1水平,以多普勒超声心动图测定指标估测肺动脉压。结果1.婴幼儿肺炎极期存在肺动脉高压,重型肺炎组重于轻型肺炎组。2.重型肺炎极期血浆irET-1升高,并与肺动脉压力是显著正相关。3.缺氧是导致血浆irET-1升高的主要原因,重型肺炎极期血浆irET-1与血氧分压呈负相关。结论婴幼儿肺炎极期缺氧可能导致内源性ET合成与释放的增加,进而引起肺动脉高压。

Abstract:To investigate the relation between plasma immunoreactive endothelin-1(irET-1) level and pulmonarty hypertension associated with infantile pneumonia.Objectives In 17 controls and 48 cases with infantile pneumonia (38 mild and 10 severe cases), plasma irET-1 levels were determined by radioimmunoassay method. A two-dimensional echocardiograph with Doppler unit was employed to estimate pulmonary arterial pressure by measurement of acceleration time (heart rate corrected ) ATc and right ventricular preejection period (RPEP) /AT.Methods 1. Pulmonary artery pressure were increased in all of the pneumonia-infants at the acute stage of the disease and the degree of the increment was related to the seventy of the Patient's condition. 2. At the acute stage of the 10 sever cases. plasma irET-1 level were elevated significantly compared with that of the 38 mild cases and the controls .and they correlated positively with the seventy of pulmonary hypertension. 3. Hypoxia is a leading cause in the increment of plasma irET-1 level and plasma irET-1 level had a negative correlation with arterial oxygen pressure at the acute stage of the 10 severe cases.Conclusion Hypoxia may probably induce endogenous endothelin increase in synthesis and release. which inturn causes pulmonary hypertension at the acute stage of pneumonia in infants and children.
