左卡尼汀对血液透析患者心功能的影响

目的 研究脑钠肽在血液透析患者心力衰竭时血浆中的水平及左卡尼汀的治疗作用.方法 检测49例血液透析心力衰竭患者透析前后脑钠肽水平,以30例无心力衰竭患者作为对照;心力衰竭组25例患者随机入组接受左卡尼汀治疗,比较治疗前后患者心功能及脑钠肽水平的变化.结果 心力衰竭组和无心力衰竭组患者透析后血浆脑钠肽水平均明显下降,差异有统计学意义（P〈0.05）;左卡尼汀组心功能改善并且脑钠肽水平下降较对照组均有统计学差异（P〈0.05）.结论 血液透析可以促使心力衰竭患者血浆脑钠肽水平下降;左卡尼汀对接受血液透析的心力衰竭者有治疗作用.

The Effect of L-carnitine to the Patients Undergoing Hemodialysis with Heart Failure

Objective To study the plasmic level of brain natriuretic peptide（BNP） and the therapeutic effect of L-carnitine in hemodialysis patients with heart failure. Methods Plasmic BNP level was detected in 49 patients undergoing hemodialysis with heart failure,and 30 cases without heart failure as control.25 of 49 patients were treated randomly with L-carnitine,and the cardiac function and BNP of all patients were assessed pre- and posttreatment. Results We found the decrease ofplasmic BNP in both the groups with or without heart failure（P 〈 0.05）. There was a significant difference in the amelioration of cardiac function and the BNP decrease after the treatment of L-carnitine（P 〈 0.05）. Conclusions Hemodialysis could reduce BNP level of the patients with heart failure. L-carnitine treatment might be effective to these patients.
