中西医结合治疗首发精神分裂症疗效观察

【目的】观察中药制剂梦醒丸合并氯丙嗪治疗首发精神分裂症的疗效及副反应。【方法】将68例首发的精神分裂症患者按入院顺序随机分为研究组（梦醒丸＋氯丙嗪）35例和对照组（氯丙嗪）33例。研究组梦醒丸18 g/d分2次口服,氯丙嗪起始剂量为50 mg/d分2次口服,根据病情调整剂量,最大量加至500 mg/d。对照组氯丙嗪服用方法和剂量同研究组。两组观察治疗3个月,分别于治疗前、1个月、2个月、3个月,使用BPRS量表进行评定分析,观察疗效。使用Tess量表结合实验室检查评定观察副反应。【结果】（1）治疗1个月时研究组有效率为85.71%,对照组有效率为51.52%。两组比较差异有统计学意义（P〈0.05）。（2）研究组和对照组BPRS总评分比较差异有统计学意义（P〈0.01）。治疗1个月时研究组和对照组BPRS评分比较差异有统计学意义（P〈0.05）研究组在显效时间上优于对照组。（3）研究组和对照组在在副反应发生的总人次分别为28和46,差异有统计学意义（P〈0.05）。【结论】中药制剂梦醒丸合并氯丙嗪治疗精神分裂症与单用氯丙嗪疗效相当。但对急性期和近期症状控制比单用氯丙嗪快。对焦虑抑郁、缺乏活力症状疗效优于单用氯丙嗪。同时可减低西药用量和减轻药物副作用。

Observation on the therapeutic effect of combined traditional Chinese medicine with Western medicine in treating initial-episode schizophrenia

【Objective】To observe the effect and side reactions of using traditional Chinese medicine（wake up pills） combined with chlorpromazine in treating initial episode schizophrenia.【Methods】Sixty-eight patients with initial-episode schizophrenia were randomly assigned to treatment group（wake up pills plus chlorpromazine,35 cases） and the control group（chlorpromazine,33 cases）.The treatment group were treated with 18g wake up pills twice per day and oral chlorpromazine（starting from least dose 50 mg twice per day,and adjusted by the severity up to the climax 500 mg per day）.The control group were treated with only chlorpromazine and the dose was same as the treated group.The patients were observed for 3 months and the effect of treatments were assessed by using BPRS scale at four time points（before treatment,the first month,the second months and the third months after treatment）.Also,the Tess scale with laboratory examination were used to evaluate the side effects.【Results】（1） The effect was different significantly between two groups（P 0.05）.after one month of treatment.（2） The difference of BPRS total score was significant（P 0.01） and the difference of after 1 month treatment was significant（P 0.05）.（3）Side reactionsthe occured in study group were 28 cases and 46 cases in control group the difference was significant（P 0.05）.【Conclusions】Chinese herbal pill combined with chlorpromazine in treating schizophrenia could speed up the short-term symptoms,decrease the dosage and with less side effects.
