2小时环孢素A血药浓度是预测移植肾排异反应的敏感指标

 目的探讨环孢素A服药后2 h的血药浓度(c₂)与临床排异及生化指标的关系。方法 选择本院58例初次肾移植术患者,在术后0-3个月内同时监测全血环孢素A的c₀和c₂各170次,并对手术前后常规生化指标进行统计学分析。结果环孢素A在非排异组和排异组的c₀与c₂分别是(371±127),(263±88)ng·mL^-1(P>0.05)和(1102±422),(727±304)ng·mL^-1(P<0.05)。结论肾移植术后c₂监测是预测和判断急性排异反应的敏感指标,术后尽早使c₂水平达到1000-14 00 ng·mL^-1,临床效果比较理想。

A sensitive predictor of renal allograft rejection:two-hour post-dose blood concentration of cyclosporine A

OBJECTIVE To study the relation among blood concentration of cyclosporine A (CsA),clinical rejection,and biochemical parameters of renal transplantation patients.METHODS Blood concentrations were monitored at 0(c₀) and 2(c₂) hour after CsA administration in 58 patients0-3 months after renal transplantation.Biochemical parameters were determined by autobiochemical analyzer.Transplantation rejection was observed by clinical features.RESULTS c₀ and c₂ of the non-rejection group (n=51) and rejection group (n=8) were (371±127) vs (263±88) ng·mL^-1(P>0.05) and (1108±422) vs (727±304 ng·mL^-1(P<0.05), respectively.CONCLUSIONS c₂ is a sensitive predictor of the clinical effects of CsA. To get satisfactory therapeutic efficacy after renal transplantation, the c₂ might be maintained in a range of 1 000-1400 ng·mL^-1.
