探讨宫角妊娠的诊治特点

目的分析总结宫角妊娠诊断及治疗的特点。方法对1998年1月至2008年9月我院收治的56例宫角妊娠患者的临床资料进行回顾性分析。结果通过症状、体征、血hCG水平及B超，术前诊断符合率50．0％。诊断宫角妊娠的主要依据是：妇检发现宫角突出伴压痛和B超提示宫角妊娠。治疗方法有药物流产、药物杀胚治疗后清官术、介入治疗、直接在B超或腹腔镜监视下清宫术、剖腹探查或腹腔镜下宫角楔形切除、宫角修补术等。其中，经腹手术占71．4％，清宫术占21．4％，急诊剖腹探查占17．9％。结论宫角妊娠因其部位的特殊，诊断有一定的困难，且存在潜在的危险，而选择恰当的治疗方法，结局良好。

Study of diagnosis and treatment features of cornual pregnancy

Objective To study the features of diagnosis and treatment of cornual pregnancy. Methods Retrospective analysis was utilized to analyze the clinical materials of 56 cases of cornual pregnancy in the 3th Affiliated Hospital of SUN Yat-sen University from January 1998 to September 2008. Results Diagnostic accordance rate before operation was 50. 0% , and the leading clues were corneal mass and tenderness and indication by ultrasound. The treatment rate of peritoneal surgery was 71.4% , uterine curettage 21.4%, and the other were received drug therapy. Conclusion There were difficulties and potential dangers in the diagnosis and treatment of cornual pregnancy, and appropriate treatment methode should be chosen for every case.
