DR与多层螺旋CT对溃疡型胃癌的诊断价值探讨

目的评价多层螺旋CT(MSCT)与DR对溃疡型胃癌的诊断价值及影响因素。方法选择经手术病理证实的溃疡型胃癌患者34例,手术前1周分别行MSCT和DR检查。所有MSCT图像(包括轴位、仿真胃镜、表面遮盖成像、透明重建)和DR图像(经后处理的多相位图像)分别由2位胃肠组放射医师实行双盲阅片,阅片结果分别与手术标本及病理比较。结果34例患者共38个溃疡中,MSCT检出35个,DR检出37个;MSCT定位正确35个,DR定位正确37个;MSCT定性正确35个,DR定性正确36个。对34个进展期胃癌的BorrmannⅡ、Ⅲ型分型,MSCT正确31个,DR正确22个。结论在溃疡型胃癌的检出、定位、定性方面,MSCT与DR无显著性差异;在BorrmannⅡ、Ⅲ型分型方面,MSCT明显优于DR。

Study on the diagnosis value of Multi-Slice CT and DR for gastric ulcerated carcinoma

Objective It is to evaluate the clinical applications of multi-slice CT(MSCT) and digital radiography(DR) and its effect factors in the diagnosis of gastric ulcerated carcinoma.Methods 34 cases patients with gastric ulcerated carcinoma proved by surgical pathology were examined by MSCT and DR in one week before operation.All the images of MSCT(including multi-planar reconstructions, CTVG,SSD,Ray sum) and DR were viewed respectively by two radiologists separately with double-blind method.The results of MSCT and DR were compared with that of postoperative pathology respectively.Results There are 38 ulcerative lesions detected by surgery and pathology,of whom 35 were detected by MSCT and 37 by DR,while 34 were localized correctly by MSCT and 37 were by DR,and 35 were diagnosed correctly by MSCT,and 36 by DR.The correct classifications of Borrmann Ⅱ and Ⅲ in 34 advanced ulcerated carcinoma were 31 on MSCT,and 22 were on DR.Conclusion There were no significant differences in the detection,location,qualitation of gastric ulcerated carcinoma between MSCT and DR,but MSCT is superior to DR in making Borrmann classification.
