肠镜联合CT检查在大肠癌术前评估中的意义

目的 探讨肠镜及CT检查对大肠癌术前评估的准确性。方法 对205例大肠癌患者进行术前肠镜、CT检查，并与手术结果比较。结果 肠镜在大肠癌的确诊、定位与术中所见及术后诊断完全相符。肠镜的Borrmann分型与术中所见的肿瘤形态基本一致；CT检查肿瘤对浆膜及周围浸润的准确率为94.5%，主淋巴结的CT阳性率为68.4%，对远处转移CT检查的阳性率为90.5%，CT未发现中间淋巴结及边缘淋巴结转移。结论 肠镜和CT检查均可在术前准确评估大肠癌肿瘤的大小、形态、部位及周围组织浸润等，对主淋巴结转移也可作一定的评估。

Significance of colonoscopy and computer tomography in preoperative evaluation of colorectal cancer

Objective To appraise the value of the preoperative colonoscopy and CT scanning in colorectal cancer. Methods The results of 234 patients with colorectal cancer by colonoscopy and CT scanning were compared with that of intraoperative findings. Results The diagnostic colonoscopy was concordant with the intraoperative findings in localization of the tumors and the clascification of Borrmann's types and morphologic features. The diagnostic accuracy of CT scanning in cancerous invasion of serosa and surrounding organs was 94.5%, that of the main group of lymph nodes and distant metastasis were 68.4% and 90.5%,respectively, but the positive rates of both intermediate and marginal lymph nodes metastasis were 0. Conclusion The morphology, size, site, and invasion of adjacent organs and tissues of colorectal cancer can be definitely demonstrated by colonoscopy and CT. CT has high accurate rate in assessing the metastasis of main group of lymph nodes.
