从“治形”与“治神”角度分析胃肠癌患者治疗规律临床研究

目的:分析胃肠癌患者在不同疾病阶段的治疗情况,从治形与治神的不同角度探索恶性肿瘤的治疗规律。方法:回顾性收集在辽宁中医药大学附属医院肿瘤科住院的的以胃癌、肠癌为第一诊断的患者,按预防复发转移期、带瘤生存早期、带瘤生存晚期、临终前期四个期分析目前治疗在各期中的对形和神应用情况。结果:收集胃肠癌患者437例,胃癌256例,结肠癌94例,直肠癌87例。带瘤生存早期和晚期手术及放化疗应用明显减少,所有患者均应用了中医抗肿瘤治疗,但在临终前期中西医均不进行抗肿瘤治疗。介入减黄和支架术等对正常形的治疗均在疾病的晚期应用。对神治疗主要包括中医治疗、减轻手术及放化疗反应、胃肠减压、骨保护、止痛、治疗肠梗阻、营养支持等几个方面。患者在预防复发转移期,当西医进行放化疗时,具有扶正和抗癌作用的中成药应用较多;在西医抗肿瘤治疗结束后,中药以抗癌为主;带瘤生存期,抗癌中药应用减少,扶正中药应用次数增加。临终前期无中药汤剂,仅予中成药静脉应用。结论:预防复发转移期对形治疗以放化疗为主,中医抗肿瘤治疗作为辅助,临终前期不进行抗肿瘤治疗。对正常形的治疗均在疾病的晚期应用。在预防复发转移期,在西医对形治疗结束后,以中药治形为主;中药对神治疗疾病全程均在应用,带瘤生存期治神中药应用增加,临终前期中药仅予以对神治疗。

Experimental Study on Effect of Treating Body and Treating Spirit of Colon Cancer Mice

Objective ： To analyze the treatment rules of gastric cancer patients in the treatment of different stages from body and spirit. Methods： We retrospectively collected gastric cancer and colorectal cancer patients for the first diagnosis in the department of oncology in Affiliated Hospital of Liaoning University of Traditional Chinese Medicine. According to the prevention of recurrence and metastasis, the survival period of early, late stage of living with the tumor, dying four periods, we analyzed the current treatments from body and spirit. Results：437 patients were collected, 256 cases with gastric cancer,94 cases with colon cancer, rectal cancer in 87 cases. In the early and late periods, operation and chemotherapy applications significantly reduced. All patients were treated with traditional Chinese medicine for anti tumor therapy. But in the last stage, traditional Chinese medicine and Western medicine are not for the anti tumor therapy. Intervention and stenting treatment on normal form is in the advanced period. Treatment from spirit mainly includes several aspects of traditional Chinese medicine treatment： reducing operation and radiotherapy and chemotherapy, gastrointestinal decompression, bone protection, relieving pain, treatment of intestinal obstruction, nutrition support. When patients are in relapse prevention period, the Western medicine is chemotherapy, with replacing and antieancer effects. The dying early period is without the decoction of Chinese medicine, but Chinese medicine only to intravenous drip. Conclusion ： The prevention of recurrence is mainly the chemotherapy and Chinese medicine anti tumor therapy is as the adjuvant therapy.
