摘    要:目的通过监测听力脑干诱发电位(Brainstem Auditory Evoked Potential,BAEP)的变化,探讨安全压力范围内高压氧(Hyperbaric Oxygen,HBO)对新生儿缺氧缺血性脑病(Hypoxic-ischemic Encephalopa-thy,HIE)早期有效治疗的压力(剂量)指标。方法在三个HBO治疗中心,对56例伴BAEP异常的重度HIE患儿,分别给予0.1、0.15、0.2MPa压力,治疗时间相同的HBO治疗,并分别于一、二、三疗程结束后和6个月龄时给予复查BAEP。结果0.15MPa和0.2MPa压力治疗组,患儿BAEP异常消失率明显高于0.1MPa压力组,定期随访在三种压力下HBO治疗的HIE患儿3个月,均无患儿并发视网膜病变(Retinopathy,RP)。结论在安全范围内,随着HBO治疗压力的升高,可能对重度HIE患儿的疗效更佳。

Abstract:Objective To investigate the value of brain stem auditory evoked potential (BAEP) for exploring the safe pressure range of hyperbaric oxygen (HBO) therapy on the newborns suffering from hypoxia-ischemic encephalopathy (HIE). Methods In three HBO treatment centre, 56 patients with severe HIE and abnormal BAEP were divided randomly into three groups. Group A was treated with the pressure of 0.1 Mpa, Group B with 0.15 Mpa and group C with 0.2 Mpa. Finally, we investigated their value of BAEP in the end of three courses of treatment and 6 months. Results The effects of therapy in group B and C are better than in group A. Having been treated, 3 months later, no patient suffering from retinopathy (RP) was discovered. Conclusion Within the safe range, the higher pressure will probably induce the better effect on newborns with severe HIE.
