不同压力的高压氧干预对HIE患儿听力脑干诱发电位的影响

目的通过监测听力脑干诱发电位(Brainstem Auditory Evoked Potential,BAEP)的变化,探讨安全压力范围内高压氧(Hyperbaric Oxygen,HBO)对新生儿缺氧缺血性脑病(Hypoxic-ischemic Encephalopa-thy,HIE)早期有效治疗的压力(剂量)指标。方法在三个HBO治疗中心,对56例伴BAEP异常的重度HIE患儿,分别给予0.1、0.15、0.2MPa压力,治疗时间相同的HBO治疗,并分别于一、二、三疗程结束后和6个月龄时给予复查BAEP。结果0.15MPa和0.2MPa压力治疗组,患儿BAEP异常消失率明显高于0.1MPa压力组,定期随访在三种压力下HBO治疗的HIE患儿3个月,均无患儿并发视网膜病变(Retinopathy,RP)。结论在安全范围内,随着HBO治疗压力的升高,可能对重度HIE患儿的疗效更佳。

THE INFLUENCE OF DIFFERENT HBO PRESSURE ON BAEP OF NEWBORNS WITH HIE

Objective To investigate the value of brain stem auditory evoked potential (BAEP) for exploring the safe pressure range of hyperbaric oxygen (HBO) therapy on the newborns suffering from hypoxia-ischemic encephalopathy (HIE). Methods In three HBO treatment centre, 56 patients with severe HIE and abnormal BAEP were divided randomly into three groups. Group A was treated with the pressure of 0.1 Mpa, Group B with 0.15 Mpa and group C with 0.2 Mpa. Finally, we investigated their value of BAEP in the end of three courses of treatment and 6 months. Results The effects of therapy in group B and C are better than in group A. Having been treated, 3 months later, no patient suffering from retinopathy (RP) was discovered. Conclusion Within the safe range, the higher pressure will probably induce the better effect on newborns with severe HIE.
