骨纤维异常增殖症的临床分析

目的探讨骨纤维异常增殖症的临床特征、诊断及治疗原则。方法对19例骨纤维异常增殖症的临床资料、影像学及病理学特点综合分析,结合临床手术术式及术后随访情况进行讨论。结果骨纤维异常增殖症好发于青少年,多位于胫骨及肋骨,临床主要表现为局部肿胀、疼痛或骨折;影像学表现为囊性、磨砂玻璃样或丝瓜络样、虫蚀状改变;组织形态学观察,正常骨结构消失,被增生的纤维组织或由不同比例的纤维性及骨性成分所取代,骨小梁排列紊乱,纤维细胞梭形,罕见核分裂像;临床术式多采用刮除或局部广泛切除。结论骨纤维异常增殖症预后良好,结合影像检查及病理检查结果可明确诊断,临床多采用局部广泛切除。

The Clinical Analysis of Fibrous Dysplasia of Bone

Objective To discuss the fibrous dysplasia of bone of the clinical features,diagnosis and treatment principles.Methods 19cases of fibrous dysplasia of the clinical data,radiological abd pathlogical featuers of an intergrated analysis of clinical surgical procedures and photoperative follow-up to discuss the situation.Results Fibrous dysplasia of bone occurs in young people,more in the tibia and ribs.The main clinical manifestalions were local swelling,pain or fractures;imaging showed cystic,frosted glass-likeor or loofah-like,insect-like change;tissue morphology,normal bone structure disappeared,was proliferation of fibrous tissue or by different proportions fibrous and bony elements replaceed by trabecular bone disordered,fiber cell spindle,rare mitotic;clinical surgical use more extensive excision curettage or local.Conclusion Fibrous dysplasia of good prognosis,combined with imaging and pathlogical finding may confirm the diagnosis,clinical and more extensive use of local excision.
