血液灌流联合血液透析治疗尿毒症临床观察

目的:观察血液灌流联合血液透析治疗尿毒症的效果。方法:将慢性尿毒症患者36例随机分为联合组20例和对照组16例,联合组采用血液灌流联合血液透析治疗,对照组只采用血液透析治疗,均连续治疗4周。治疗前后采血测定血清尿素氮(BUN)、血清肌酐(Scr)、β2-微球蛋白(2-MG)等指标,并观察两组皮肤瘙痒、睡眠障碍、血压等症状改善程度。结果:治疗后联合组β2-MG明显下降,差异有显著性(P<0.01);而对照组β2-MG无明显变化(P>0.05);治疗后两组的BUN、Scr均有明显下降,差异有显著性(P<0.01),但两组的BUN和Scr下降幅度无明显差异。联合组皮肤瘙痒症状改善显著好于对照组(P<0.05)。结论:血液灌流联合血液透析(HP/HD)能有效清除尿毒症患者体内的毒素,是治疗尿毒症的有效方法,可提高生存质量。

Clinical observation of hemoperfusion combined with hemodialysis in the treatment of uremia

Objective:To observe effects of hemoperfusion combined with hemodialysis in the treatment of uremia.Methods:36 patients with chronic uremia were randomly divided into the combined group(20 cases) and the control group(16 cases).The combined group used hemoperfusion combined with hemodialysis,and the control group only used hemodialysis,all treatment for 4 weeks.Determinated two groups of blood urea nitrogen(BUN),serum creatinine(Scr),serum 2-microglobulin(2-MG) and other indicators before and after treatment,observed improve situations of two groups of skin pruritus,sleep disorders,blood pressure and other symptoms.Results:After the treatment,serum 2-MG in the combined group decreased significantly,there was significant difference(P<0.01);while in the control group there was no significant change(P>0.05).After the treatment,BUN and Scr in both group were significantly reduced,there were significant difference(P<0.01).However,there was no significant difference between the two groups in the decline.The combined group was significantly better than the control group in the improvement of skin pruritus(P<0.05).Conclusion:Hemoperfusion combined with hemodialysis can effectively remove uremic toxins.It is effective method for treatment of uremia,and can improve the quality of life of patients with uremia.
