十二指肠溃疡穿孔254例治疗体会

目的比较十二指肠溃疡穿孔各种治疗方法的近期疗效。方法对我院 1 994年 1月至 2 0 0 3年5月间所收治的十二指肠溃疡穿孔病人的临床资料进行回顾性分析。结果在肛门排气时间、住院时间、住院费用、并发症发生率方面 ,非手术组与手术组相比有显著性差异 (P <0 .0 5 ) ,穿孔修补术组与根治性手术组相比亦有显著性差异 (P <0 .0 5 )。结论只要严格掌握适应症 ,十二指肠溃疡穿孔非手术治疗是合理和安全的 ,如确需手术 ,首选穿孔修补术 ;在有条件的医院 ,腹腔镜修补术是目前比较理想的术式

The Treatment Experience of 254 Patients with Duodenol Ulcer Perforation

Objective To compare the short-term effects of various treatments of gastroduodenal ulcer perforation.Methods A retrospective analysis of 254 patients with duodenal ulcer perforation from Jan.1996 to may.2003 in our hospital was made. Results There were significant differences in the time of passing flatus, hospital stay, medical cost, and complication rate between the non-operation group and the operation group, and between the repair group and the radical group. Conclusion Nonclusions Non-operation treatments were safer and more reasonable for duodenal ulcer perforation if they were used in strict indication. If the patient must be operated on, closure of perforation is recommended. Laparoscopical closure may be the best surgical choice.
