经尿道膀胱肿瘤切除术联合髂内动脉栓塞化疗治疗膀胱癌

目的评价髂内动脉栓塞化疗配合经尿道膀胱肿瘤切除术(TURBt)治疗膀胱癌的临床效果。方法将经TURBt且病理证实的50例膀胱癌患者分为术前栓塞组(22例)和单纯手术组(28例)。术前栓塞组经髂内动脉栓塞化疗后2~5d内接受手术。结果术前栓塞组在栓塞后至手术前血尿即减轻或消失,术中出血减少,膀胱镜检查发现肿瘤缩小,肿瘤及周围组织有散在的陈旧性出血与小血管内血栓形成,肿瘤表面坏死。1年内的复发率比较差异无统计学意义(P>0.05),3年内的复发率为9.09%,明显低于单纯手术组(32.14%),副作用比较差异无统计学意义(P>0.05)。结论术前髂内动脉栓塞化疗可缩小膀胱癌病灶,降低复发率,提高生存率,操作简单,安全有效。

Transurethral resection of bladder tumor plus intra-internal iliac-arteria infusion chemotherapy and embolization for the treatment of bladder cancer

Objective To evaluate the clinical efficacy of intra-internal iliac-arteria infusion chemotherapy and embolization plus Transurethral resection of bladder tumor (TURBt) for the treatment of bladder cancer. Methods A total of 50 cases of bladder cancer was confirmed by TURBt.The cases were divided into preoperative transarterial embolization group (n=22 cases ) and operation alone group (n=28 cases).Surgery was performed in 2-5 days after transarterial embolization.Result The hematuria alleviated or disappeared after the patients received transarterial embolization preoperatively,and the hemorrhage also became less during the operation.The reduction of cancer volume,clotted by thrombus in the blood vessels of the cancer or circumferential tissue were found with the cystoscopy.The tumor recurrence rate had no obvious difference in a year( P>0.05),but had obvious difference in three years ( P<0.05),which was obviously lower than in operation alone group(32.14%).The adverse effects had no obvious difference ( P>0.05).Conclusion Intra-internal iliac-arteria infusion embolization for bladder cancer can reduce the tumor size,decrease the recurrence,and improve the survival rate,which is simple and safe to maneplate.
