广州市某社区初产妇产后抑郁情况及其影响因素研究

目的了解广州市初产妇在产褥期、产后第3个月以及产后第6个月的抑郁状况,并探讨其抑郁症状产生的影响因素。方法采用整群抽样法,对广州市某社区卫生服务中心接受产后保健服务的506名产妇的一般情况及抑郁状况进行问卷调查。结果调查对象产褥期抑郁发生率为30.43%,其影响因素有＂婴儿生长是否良好＂和＂是否自然分娩＂ 产后第3个月抑郁发生率为23.80%,其影响因素有年龄、＂家中有无老人同住＂和＂是否母乳喂养＂ 产后第6个月抑郁发生率为11.79%,其影响因素有年龄、＂婴儿生长是否良好＂以及＂是否恢复上班＂。结论广州市初产妇产后抑郁的影响因素呈现随时间而改变的趋势,提示在社区产后保健工作中,应根据产后的不同时间段,针对不同的危险因素,筛选出产后抑郁的高危人群,并提供有针对性的干预。

Status of postpartum depression among primiparas in one community in Guangzhou City and relevant influencing factors

Objective To understand the status of postpartum depression among primiparas in one community in Guangzhou during puerperium, 3rd and 6th months after parturition, and to identify its influecing factors. Methods With muster sampling, a survey was conducted among 506 primiparas in a community health center in Guangzhou. Results Dur- ing puerperium, 30. 43% of the primiparas got the EPDS score over 9, and its influecjng factors included ＂the situation of baby＇ s growth＂ and ＂style of parturition＂ . During 3rd month, 23.80% of the primiparas got the EPDS score over 9, and its influecing factors included age, ＂whether to live with elderly or not＂ and ＂ whether to breastfeed the child or not＂ . During 6th month, 11.79% of the primiparas got the EPDS score over 9, and its influecing factors included age, ＂the situ- ation of baby＇ s growth＂ and ＂whether to go back to work or not＂ . Conclusion In different time after parturition, there were different influecing factors to postpartum depression among primiparas in Guangzhou. And the prevention should be taken according to these situations.
