迟发性外伤性颅内血肿手术治疗36例的临床分析

目的探讨迟发性外伤性颅内血肿早期诊断及手术治疗指征。方法回顾性分析了我科室2004年5月至2008年7月收治的52例迟发性外伤性颅内血肿,手术36例,其中硬膜外血肿6例,硬膜下血肿10例,脑内血肿17例,多发性血肿3例。结果手术清除血肿36例,随访半年GOS良好20例,中残4例,重残5例,死亡7例。结论迟发外伤性颅内血肿早期临床表现较首次CT图像显示脑损伤严重的多,对有明显占位的迟发性颅内血肿尽早手术清除血肿。

Operative treatment of delayed traumatic intracranial hematomas in 36 cases

Objective：To investigate the early diagnosis and surgical indications of delayed traumatic intracranial hematomas.Methods：The clinic data of 52 cases with delayed traumatic intracranial hematomas in our department from May 2004 to July 2007 were reviewed retrospectively.36 patients received operative treatment.Rusults：36 cases undergone the operation were followed up 6 months,The glasgow outcome scale（GOS） showed that 20 recovered well,4 moderate disability,5 severe disability,7 died.Conclusions：The early clinical manifestation of delayed traumatic intracranial hematomas is more severe than the brain injury diagnosed by CT scans at first.The delayed traumatic intracranial hematomas with obvious space-occupying lesion should be removed by operation as early as possible.
