高频震荡通气治疗新生儿肺透明膜病

摘    要:目的:评价高频振荡通气治疗新生儿肺透明膜病的疗效。方法:将32例新生儿呼吸窘迫综合征患儿分成两组,16例采用高频振荡通气治疗,16例常规机械通气治疗;观察两组患儿的肺功能及并发症。结果:高频振荡通气组患儿在治疗后6,12,24h,氧浓度、氧合指数明显下降并低于机械通气组,动脉/肺泡氧分压比值明显上升并高于机械通气组,有显著性差异(P〈0.05或0.01)。高频振荡通气组并发症和病死率低于机械通气组,存活的患儿的上机时间、氧疗时间、住院时间比机械通气组短,差异有显著性(P〈0.05或0.01)。结论:高频振荡通气能更好改善肺透明膜病患儿的肺氧合功能,减少并发症,缩短病程,降低病死率,对治疗具有很好的疗效。

Abstract:Objective To study the effect of high-frequency oscillatory ventilation to newborn infants with neonatal hyaline membrane disease.WTHZ]Methods Thirty-two infants with neonatal hyaline membrane disease were divided into 2 groups,16 cases treated with high-frequency oscillatory ventilation and 16 cases treated with conventional mechanical ventilation. Results The mean fractional inspired oxygen and oxygenation index were decreased,and arterial/alveolar PO_2 was increased after treatment with high-frequency oscillatory ventilation from 6 to 24 hours in high-frequency oscillatory ventilation group,which was significantly different compared with the conventional mechanical ventilation((P<0.05,P<0.01)).The time of mechanical ventilation and hospitalization was shorter in high-frequency oscillatory ventilation group((P<0.05)),and complications and mortality rate were low((P<0.01)).Conclusion High-frequency oscillatory ventilation therapy may result in a rapid improvement in oxygenation,and shorten the duration of mechanical ventilation hospitalization time,reduce the complication and mortality rate in newborn infants with neonatal hyaline membrane disease.
