腰椎手术失败综合症的CT表现

目的探讨腰椎手术失败综合症的CT表现及其与临床症状的关系。方法回顾性分析2009年1月至2011年5月共21例行椎板切除手术后失败综合症患者的临床症状及CT检查的影像学资料。结果硬膜外瘢痕及粘连16例、硬脊囊受压7例、椎管及侧隐窝狭窄13例、椎间盘再膨出或突出7例及椎体滑脱2例、无阳性发现2例。结论硬膜外瘢痕粘连形成是术后失败综合症的主要原因,薄层CT及多平面重建可清楚了解腰椎椎板侧切手术后椎间盘及椎管情况,尤其是对硬膜外瘢痕的定位及观察具有较大的意义,可为临床治疗提供重要的影像信息。

CT feature of Failed Back Surgery Syndrome

Objective To investigate CT feature of Failed Back Surgical Syndrome and the relationship between its symptoms and imaging manifestation.Methods The clinical symptoms and CT images of 21 patients with failed back surgery syndrome between January 2009 and May 2011 were retrospectively studied.Results epidural scar and conglutination(n=16),dural sac compression(n=7),stenosis of vertebral canal and lateral recess(n=13),intervertebral discs reherniation or protrusion(n=7),vertebral olisthy(n=2)and no any positive appearance(n=2).Conclusions The formation of epidural scar and conglutination is the main reason for FBSS.The condition of intervertebral disc and vertebral canal after laminectomy can be displayed by thin layer CT and MPR.It is greatly useful in the location and observation of epidural scar,which also can provide important imaging information for clinical treatment.
