眼睑下垂重症肌无力患者视频眼像测算系统的分析

摘    要:目的应用测量新斯的明注射后重症肌无力(MG)眼睑下垂程度变化,为临床诊断提供客观、精确和规范化的测量方法。方法本文研究包括34名MG病例、30例非MG患者及30名正常健康个体。注射新斯的明试验前和试验后,测量MG组、非MG对照组和正常对照组病人睑裂大小,分析MG组和对照组的多次测量值和显效时相对值,计算多次测量值的敏感度、特异度和约登指数。结果 MG组患者、非MG对照组和正常对照组平视睑裂大小多次测量值的平均数±标准差分别为2.621±0.376mm、0.823±0.266mm和0.853±0.254mm。MG组与非MG组和正常组间差异有统计学显著意义(P〈0.05)。多次测量值的灵敏度、特异度和约登指数分别96.7%、100%、和0.967。结论应用视频眼像测算系统进行新斯的明试验辅助诊断MG,是客观、精确和规范化的测量方法 。

Abstract:Objective In order to provide a direct, accurate and standard measuring method for clinical diagnosis of myasthenia gravis (MG,), we recorded the changes of eyelid drooping degree after the injection of neostigmine with a video eye-measuring system. Methods Thirty-four patients with MG, 30 cases with non-MG disease and 30 normal helthy subjects were included in this study. The palpebral fissure size of patients in the MG group, non-MG control group and normal control group before and after the injection of neostigmine, analyzing the repeated value and effectively relative value, and then calculated the sensitivity, specificity and Youden index of the repeated values. Results The mean ± standard deviation value of palpebral fissure size of orthophoria was 2.621±0.376 mm, 0.823±0.266 mm and 0.853±0.254 mm in patients of the MG, non-MG control and normal control groups, respectively. There was a significant difference (P0.05) among patients of the MG, non-MG control and normal control groups. The sensitivity, specificity and Youden index of the repeated values was 96.7%, 100% and 0.967, respectively. Conclusions The application of video eye-measuring system with the neostigmine test is a direct ,accurate and standard method for the diagnosis of MG.
