CT引导经皮胸部病变切割针穿刺及常见并发症

目的：探讨如何提高CT引导经皮胸部病变切割针穿刺术的准确性、安全性,降低并发症。方法：选择2001年3月～2005年8月的68例经皮胸部穿刺术患者。结果：68例中穿刺病理确诊恶性肿瘤45例,良性病变21例,未出结果2例。手术32例,与手术病理一致31例,无假阳性。经手术和随访证实,假阴性4例,假阴性率18.18%（4/22）。阳性和阴性预测值分别为100%（45/45）和81.82%（18/22）。气胸11例,占16.18%（11/68）,无插管处理。沿穿刺通道少量出血18例,咳血6例。结论：CT引导经皮胸部穿刺术准确性较高,并发症较低,应按病变部位选择穿刺体位和穿刺方法;熟悉病理解剖学知识后熟练操作,可降低常见并发症的发生。

CT - guided Percutaneous Biopsy of Pulmonary Lesion and Complications

Objective:To discuss accuracy,safety and less complication of CT-guided percutaneous transthoracic needle biopsy of pulmonary lesion.Methods: The procedure was performed in 68cases with chest masses,during March 2001August 2005.Results: Of 68 cases studied,45 proved to be cancers.21 cases were benign lesions,2 cases were no results,no false positive.4 cases were false negative confirmed by surgery and clinical course.The positive and negative prediction value were 100%(45/45) and(18/22),respectively.11cases occurred pneumothorax 16%(11/68) without chest tube drainage.18cases occurred little blood along needle biopsy thoroughfare,6 cases coughed with blood.Conclusion: CT-guided predaceous transthoracic biopsy is useful in diagnosing pulmonary lesion with high accuracy and few complications.
