经食管三维超声心动图的临床应用价值探讨

摘    要:目的探讨经食管三维超声心动图(3D-TEE)的临床应用价值及应用前景。方法选择各类心脏病患者20例。包括冠心病非心肌梗死1例、心肌梗死合并室壁瘤3例、先心病9例、瓣膜病7例。17例为术中体外循环前采集图像,3例为常规经食管超声检查时采集。结果大部分患者可获得满意的三维图像,并显示部分二维超声无法显示的结构及关系,同时由于经食管超声改善了经胸超声图像清晰度及分辨率,所显示三维图像也较经胸三维超声图像(3D-TTE)更为清晰。结论3D-TEE可立体动态显示心脏的解剖结构及毗邻关系、运动、功能等,具有着较高的应用价值和较好的应用前景。

Abstract:Objective To explore the clinic application of three-dimensional transesophageal echocardiography (3D-TEE) in cardiac diseases.Methods Twenty patients with different cardiac diseases were selected for this study,including one case with coronary heart disease without myocardial infraction,three cases with old myocardial infraction with left ventricular aneurysm,nine cases with congenital heart diseases,seven cases with valvular heart diseases.3D-TEE images were acquired in the operative rooms before extracorporeal circulation,except for 3 cases performed in regular TEE room.Results 3D-TEE images were satisfied in nearly all cases.Compared with 2-dimensional transesophageal echocardiography (2D-TEE),3D-TEE could reveal some cardiac structures and their spatial relationships that 2D-TEE was unable to show.Meanwhile,3D-TEE could provide better image quality than that of transthoracic 3-demensional images,due to the high frequency of transesophageal probe and close related position between esophageal and the heart.Conclusions 3D-TEE could dynamically show three-dimensional cardiac structures,movement and function precisely,as well as the position and contiguous relations of abnormal anatomical structure,etc.3D-TEE can be a useful tool for clinical diagnosis and monitoring guidance of cardiac treatment.
