| 摘 要： | 目的:了解门诊药房退药原因,促进医院合理用药。方法:收集我院2010年1-5月门诊药房退药处方共276张,以《医疗机构药事管理暂行规定》为依据,从退药科室、退药原因、退药品种等角度进行统计、分析,并提出建议。结果与结论:退药最多的科室为心内科(96张,34.8%),其次为内分泌科(40张,14.5%);退药原因包括医师开错药(110张,39.86%)、用法用量错误(22张,7.97%)等;所退药品以抗微生物类药物(69张,25.0%)、循环系统用药(55张,19.9%)为主。为降低门诊药房退药率,医院相关部门应建立退药制度与奖惩制度;加强法规宣传与职业教育;加强医患沟通,提高患者用药依从性;加强药房管理与"医、药"沟通;开展药师用药咨询和指导工作等。
