腹腔镜全子宫切除术与开腹全子宫切除术的临床价值分析

目的分析腹腔镜全子宫切除术（TLH）与开腹全子宫切除术（TAH）的临床价值。方法采用随机数字表法将符合全子宫切除术指证的99例患者分为TLH组49例和TAH组50例,分析两种术式的手术疗效、对卵巢功能影响和手术并发症情况。结果 TLH组在术中出血量、排气时间、下床时间和住院时间方面明显优于TAH组（P〈0.01）;TLH组对卵巢功能影响与TAH组比较相对较小（P〈0.05）;TLH组手术并发症发生率4.08%明显低于TAH组24.00%（P〈0.01）。结论 TLH术具有术中出血量少、胃肠功能恢复快和手术并发症发生率低等优势,在促进患者早期功能恢复和延缓卵巢功能衰竭方面更具优势,值得临床对该手术方式继续研究和探讨。

Analysis of the clinical value of laparoscopic hysterectomy and abdominal hysterectomy surgery

Objective To analyze the clinical value of laparoscopic hysterectomy（TLH） versus open resection（TAH）.Methods 99 patients were divided into 50 cases in the TLH group and 49 patients in TAH group,the efficacy of two surgical procedures,the impact on ovarian function and complications were analyzed.Results TLH blood loss,exhaust time,get out of bed time and hospital stay was significantly better than the TAH group（P〈0.01）;impact of the the TLH group of ovarian function with a relatively small group of TAH（P〈0.05）;TLH group surgical complication rate of 4.08% was significantly lower than the by the TAH group 24.00%（P〈0.01）.Conclusion TLH surgery with less blood loss,gastrointestinal function recovery faster and surgical complication rate is low and other advantages,in pro moting early functional recovery in patients and slow advantage ovarian failure,worthy of clinical study of surgical methods continue discussed.
