多普勒超声在深静脉血栓诊断中的应用

目的探讨彩色多普勒超声在下肢深静脉血栓诊疗中的应用价值。方法 2010～2012年选择临床初步诊断为下肢深静脉血栓形成的患者60例为研究对象。采用彩色多普勒超声进行下肢探测,观察记录血栓形成的位置、形态、受累静脉的管径、血流情况。所有患者同时行数字减影血管造影（DSA）检查作为金标准,将彩超结果与DSA结果进行比较,评价彩色多普勒超声在下肢深静脉血栓诊疗中的灵敏度、特异度、准确性等指标。结果 60例患者中急性、亚急性静脉血栓患者22例,慢性期38例。中央型血栓5例,周围型22例,混合型33例。与DSA比较,CDFI对股静脉血栓的诊断灵敏度最高,96.8%,对腘静脉的血栓诊断的特异度最高,92.3%,对股静脉血栓诊断的正确率最高,93.8%。结论彩色多普勒超声用于诊断下肢深静脉血栓具有无创伤、便捷、可重复等优点,对股静脉、腘静脉血栓诊断的灵敏度、特异度、准确率较高。

Application of doppler ultrasound in diagnosis of deep venous thrombosis

Objective To discuss application of doppler ultrasound in diagnosis of deep venous thrombosis.Methods From 2010 to 2012,60 cases with deep venous thrombosis were selected as subjects.Doppler ultrasound was used to detected the deep veins of lower limb.Location,shape of thrombosis,diameter of affected vein,blood flow were ob served.DSA results as golden standard,doppler ultrasound results were compared with DSA results,and sensitivity,specificity,veracity were evaluated.Results In 60 cases,Acute and sub-acute deep venous thrombosis was 22 cases,chronic 38 cases.Central type was 5 cases,peripheral was 22 cases,mixed was 33 cases.Compared to DSA,sensitivi ty of CDFI in diagnosis of femoral vein thrombosis was highest,93.8%;specificity in diagnosis of popliteal venous thrombosis was highest,92.3%;veracity of CDFI for diagnosis of femoral vein thrombosis was highest,93.8%.Conclusion CDFI in diagnosis of deep venous thrombosis shows noninvasive,convenient and repeatable.Sensitivity,specificity,veracity of CDFI in diagnosis of femoral vein thrombosis and popliteal venous thrombosis are high.
