镜下镍钛合金取石篮配合钬激光碎石治疗输尿管上段结石98例

目的探讨输尿管镜下镍钛合金取石篮配合钬激光碎石术治疗输尿管上段结石的临床疗效。方法采用输尿管镜下镍钛合金取石篮配合钬激光碎石术治疗输尿管上段结石98例。结石直径0.6-2.5 cm,平均1.2 cm。结果单次碎石成功率96%（94/98）。2例术中改开放手术;1例结石上移入肾内,术后结合体外震波碎石治愈;1例肾内残石,未作处理。无输尿管穿孔、撕裂等并发症。结论镍钛合金取石篮配合钬激光治疗输尿管上段结石,有效防止了近肾盂处结石上移入肾、碎石过程中破碎的结石块流入肾盂,提高了成功率,降低了肾内残石发生率,是一种较好的腔内碎石方法。

Treatment of Upper Ureteral Stones With NITINOL Stone Entrapment and Extraction Device Combined With Holmium:YAG Laser Under Ureteroscopy

Objective To evaluate the effectiveness of NITINOL stone entrapment and extraction device combined with pneumatic lithotriptor under urctcroscopy for managing upper uretcral stones. Methods 98 patients with upper ureteral stones were treated with NITINOL stone entrapment and extraction device combined with pneumatic lithotriptor under ureteroscopy .The diameter of the stones was 0.6～2.5 cm. Results 96%（94/98） stones were fragmented for one session of treatment.1 case were pushed upward then undergoing extracorporeal shock wave lithotripsy（ESWL） after surgical operations.Operations in 2 cases had to be changed to open surgery. Conclusion Treatment of upper ureteral stones with NITINOL stone entrapment and extraction device combined with Holmium：YAG laser under ureteroscopy has advantages of high safety,less complication and satisfactory efficacy.
