摘    要:目的探讨体感诱发电位(SEP)检测在小儿脑性瘫痪(cerebralpal-sy,CP)诊断中的临床应用价值。方法按国际脑电图学会推荐标准,以180例0~14岁各年龄组小儿SEP正常值为对照组,对77例4月~14岁临床诊断CP患儿进行正中神经及胫后神经SEP检测,并与CT、EEG结果进行比较分析。结果CP患儿中SEP异常率(66/77)86%,CT异常率(37/77)48%,EEG异常率(16/38)42%。37例CT正常者中SEP异常率为(30/40)75%。CP患儿SEP改变上肢以刺激正中神经N20波潜伏期延长、下肢以刺激胫后神经P40波潜伏期延长为明显。结论SEP在检测脑瘫时有较高的阳性率,是早期、客观诊断脑瘫的一种可靠方法。

Abstract:Aim To explore the diagnostic value of so matosensory evoked potentials(SEP)for cerebral palsy in pediatrics.Method According to the technology index set by the Internat ional 10-20System of EEG Electrode Placement,77children with cerebra l palsy(aged from 4months to 14years old)were checked with SEP in their median nerve and tibial nerve,compared with the normal data of SEP in 180heal thy children(aged 0-14years old).Comparison and analysis were made in the result of CT and EEG.Result Among the 77subjects with cerebral palsy,abnormal SEP was among 66out of 77patients with cerebral palsy(86%),abnormal CT rate was 48%(37/77)and abnormal EEG rate was 42.1%(16/38.Among 37normal cases of CT,the abnormal rate of SEP was 75%(30/40).The change of SEP was evidently demonstrated by the extended latent period of the wave N20and wave P40aft er stimulating the median nerve of arms and posterior tibial nerve of legs.Conclusion SEP shows a high rate of abnormality i n checking the cerebral palsy,which is regarded as one of the reliable methods for diagn osing cerebral palsy objectively and prom ptly.
