伴与不伴精神病性症状的抑郁症临床特征的对照研究

目的探讨伴与不伴精神病性症状(Psychoticsymtoms,PS)的抑郁症患者临床特征的差异。方法对64例伴PS的抑郁症患者(研究组)与118例不伴PS的抑郁症患者(对照组)的人口学特征、主要临床症状及治疗情况进行对照分析。结果研究组的阳性家族史、焦虑激越、自责自罪、绝望和自杀行为等出现率显著高于对照组(P<0.05或P<0.01),研究组的自杀危险性为对照组的2.2倍。结论伴PS的抑郁症可能是抑郁症的一个独特的亚型,通常需要抗抑郁药物与抗精神病或电痉挛(ECT)联合治疗。

A Comparative Study on Clinical Features of The Depression with Psychotic Symptoms

Objective To explore the difference in clinical features between depressive pativents with Psychotic Symptoms(PS)and nonpsychotic symptoms.Method64cases of major depressive patients with PS were compared with118cases with non-PS.Results There significant differences in the frequencies of psychotic famly history,agitation,self-guity,despair and suicidal behavior between patients with PS and patients without PS(P<0.05or P<0.01),the risk of suicide in patients with PS was2.2times as much as in those of non PS.Conclusion Depression with PS may be a specific type of the major depressive disorder,and combined treatment is always necessary to these patients.
