脊髓型颈椎病前路手术早期并发症相关因素分析(附258例报告)

目的 探讨影响颈椎病前路手术早期并发症发生的相关因素。方法 回顾1992年1月~2003年12月间脊髓型颈椎病前路手术258例,选取性别、年龄、术式等指标进行统计,对其进行单因素筛选、多因素Logistic回归分析。结果 共发生并发症19例(20例次),发生率为7.4%;单因素分析显示与颈椎前路手术早期并发症相关的主要因素有瘫痪情况和手术方式,多因素Logistic回归分析显示瘫痪情况是决定术后早期并发症发生的主要因素。结论 瘫痪情况对颈椎病前路手术早期并发症有显著影响,做好患者围手术期处理可以降低术后早期并发症的发生率。

Risk factor analysis of early complications following anterior spine surgery in 258 cases of cervical spondylotic myelopathy

Objective To define the possible risk factors for early complications following anterior surgery for cervical spondy- lotic myelopathy. Methods Two hundred and fifty-eight cases of cervical spondylotic myelopathy receiving anterior surgery between January 1992 and December 2003 were reviewed and the gender, age and other factors of the patients were analyzed with single-factor analysis and multiple logistic regression. Results Complications arising in the early stage after the surgery were found in 19 cases (20 events), with an incidence rate of 7.4%. Single-factor analysis showed that the two factors, paralysis and surgical approaches, were correlated with the early complications, and paralysis was identified as the major factor determining the occurrence of these complications by multiple logistic regression. Conclusion Paralysis is the major factor related to the occurrence of early complications following anterior surgery for cervical spondylotic myelopathy, which reiterates the impor- tance of proper perioperative management.
