化痰解毒散结汤联合CHOP方案治疗恶性淋巴瘤的临床效果及对患者免疫功能和生存质量的影响

目的 探讨化痰解毒散结汤联合CHOP方案治疗恶性淋巴瘤的临床效果及对患者免疫功能和生存质量的影响.方法 将2011年1月至2015年1月于本院接受诊治的恶性淋巴瘤患者78例随机分为研究组与对照组,各39例.研究组采用化痰解毒散结汤联合CHOP方案治疗,对照组仅采用CHOP方案治疗.两组均以化疗3个周期为观察时间.分析两组化疗疗效、化疗前后KPS评分、化疗前后T细胞亚群变化及化疗期间毒副反应发生情况.结果 研究组RR(69.23％)明显高于对照组(48.72％)(P＜0.05);研究组生存质量提高率明显高于对照组(P＜0.05);研究组生存质量下降率明显低于对照组(P＜0.05);研究组治疗后CD8+明显低于治疗前,CD3+、CD4+、CD4+/CD8+明显高于治疗前(P＜0.05);研究组治疗后CD8+明显低于对照组,CD3+、CD4+、CD4+/CD8+明显高于对照组(P＜0.05);研究组白细胞减少、淋巴细胞减少、周围神经损害及粒细胞减少发生率明显低于对照组(P＜ 0.05);两组血小板减少、恶心呕吐、肝功能损伤发生率比较差异无统计学意义(P＞0.05).结论 化痰解毒散结汤联合CHOP方案治疗恶性淋巴瘤临床效果显著,可明显提高患者生存质量和免疫功能,值得临床研究.

Phlegm-reducing,detoxifying, and lump-dissolving decoction combined with CHOP regime in treatment of malignant lymphoma and its effect on immune function and quality of life

Objective to investigate the clinical efficacy of phlegm-reducing,detoxifying,and lumpdissolving decoction combined with CHOP regime in the treatment of malignant lymphoma and its effect on the patients' immune function and quality of life.Methods 78 patients with malignant lymphoma treated at our hospital from January,2011 to January,2015 were randomly divided into a study group (n=39) and a control group (n=39).The study group were treated with phlegm-reducing,detoxifying,and lump-dissolving decoction and CHOP regime and the control group CHOP regime.Both groups were observed 3 cycles.The chemotherapy effects,the KPS scores and the changes of the subsets ofT-cells before and after chemotherapy,and the incidences of toxic and adverse reactions during chemotherapy in both groups were analyzed.Results The RR rate were significantly higher in the study group than in the control group(69.23％ vs.48.72％,P＜0.05).The increase rate of living quality was significantly higher and the decrease rate of living quality was significantly lower in the study group than in the control group (P＜0.05).The CD8+ was significantly lower and CD3+,CD4+,and CD4+/ CD8+ were significantly higher after than before the treatment in the study group and in the study group than in the control group after the treatment (P＜0.05).The decreased rates of leukocyte,lymphocytes,and granulocytes and the incidence peripheral nerve damage were significantly lower in the study group than in the control group (P＜0.05).There were no statistical differences in the incidences of thrombocytopenia,nausea and vomiting,and hepatic injury between these two groups (P＞0.05).Conclusions Phlegm-reducing,detoxifying,and lumpdissolving decoction combined with CHOP regime in the treatment of malignant lymphoma is quite effective and can improve the patients' immune function and quality of life,so it is worth being clinically studied.
