心率变异性指标评估急性脑卒中患者心脏自主神经功能的应用价值探讨

摘    要:目的 分析并探讨心率变异性指标(HRV)评估急性脑卒中患者心脏自主神经功能的应用价值.方法 选取急性脑卒中患者90例为观察组,非心脑血管疾病患者90例为对照组.检测患者动态心电图,记录患者24h的心电信号,分析对比两组心率变异指标.结果 观察组患者窦性R-R间期的标准差(SDNN)、每5min时段窦性R-R间期平均值标准差(SDANN)、rMSSD以及PNN50明显低于对照组(P<0.05).观察组脑梗死患者和脑出血患者SDNN、SDANN、窦性R-R间期的平方根(rMSSD)以及相邻R-R间期互差>50ms的心跳数所占分析信息期间内心搏数百分比(PNN50)比较差异无统计学意义(P>0.05).结论 心率变异指标可以有效预测急性脑卒中患者心脏自主神经功能变化,对监测患者心脏功能障碍程度有一定意义,可在一定程度上避免心源性猝死事件发生.

Discussion on the application value of heart rate variability indexes in evaluating cardiac autonomic nervous function in patients with acute cerebral stroke
Abstract:Objective To analyze and discuss the application value of heart rate variability (HRV) indexes in evaluating cardiac autonomic nervous function in patients with acute cerebral stroke.Methods 90 cases of acute cerebral stroke were selected as observation group,and 90 patients without cardiovascular or cerebrovascular diseases were selected as control group.The dynamic electrocardiogram of the patients were detected,24 h electrocardiosignal of the patients were recorded,and HRV indexes between two groups were analyzed and compared.Results SDNN,SDANN,rMSSD,and PNN50 of observation group were significantly lower than those of control group (P<0.05).There were no statistically significant differences in SDNN,SDANN,rMSSD,and PNN50 between patients with cerebral infarction and patients with cerebral hemorrhage in observation group (P>0.05).Conclusions HPV indexes could effectively predict cardiac autonomic nervous function changes in patients with acute cerebral stroke,which showed certain significance to monitor the severity of cardiac dysfunction in patients,and could avoid sudden cardiac death events to some extent.
