小夹板固定中不同固定位置选择对Colles骨折术后关节稳定性及腕关节功能的影响

目的 探讨小夹板固定中不同固定位置选择对Colles骨折术后患者关节稳定性及腕关节功能的影响.方法 选取本院骨科Co11es骨折患者共70例,根据小夹板固定位置不同随机分为观察组(旋后位)和对照组(旋前位),观察治疗前、复位后、随访6周掌倾角、尺偏角、桡骨短缩长度,评估腕关节功能.结果 治疗前及复位后,两组掌倾角、尺偏角、桡骨短缩长度比较,差异无统计学意义(P＞ 0.05);治疗后第6周,观察组掌倾角、尺偏角及桡骨短缩长度丢失均小于对照组(P＜ 0.05);治疗后第6周,观察组腕关节功能优良率(82.86％)较对照组(71.43％)高(P＜ 0.05).结论 使用小夹板将患肢固定在腕关节掌屈尺偏前臂旋后位能恢复患处关节生理结构,稳定持久,便于腕关节功能恢复,值得推广.

Influence of fixation in different positions with small splint on stability and function of wrist joint after surgery for Colles fracture

Objective To explore the influence of fixation in different positions with small splint on stability and function of wrist joint after surgery for Colles fracture.Methods 70 patients with Colles fracture treated at the department of orthopaedics at our hospital were randomly divided into an observation group (supination) and a control group (pronation) according to different fixation positions.Before the treatment and after restoration and 6 weeks' follow-up,the dorsal tilt,ulnar inclination,radial shortening,and wrist joint function were observed.Results Before the treatment and after restoration,there were no statistical differences in the dorsal tilt,ulnar inclination,and radial shortening between these two groups (P ＞ 0.05).6 weeks after the treatment,the dorsal tilt,ulnar inclination,and radial shortening lost less in the observation group than in the control group (P ＜ 0.05);the good and excellent rate of wrist joint function was 82.86％ in the observation group and was 71.43％ in the control group (P ＜ 0.05).Conclusion Fixation in supination position can restore the patients,joint physiological structure and is stable for long,helpful for the recovery of wrist joint function,and worth being generalized.
