早期机械通气联合一氧化氮吸入治疗新生儿重度呼吸衰竭的疗效分析

摘    要:目的 探讨早期机械通气联合一氧化氮吸入治疗新生儿重度呼吸衰竭的疗效.方法 收集我院2014年6月至2015年6月入院的100例重度呼吸衰竭患儿随机分为两组,对照组患儿采用早期机械通气治疗,实验组加用一氧化氮吸入治疗,比较两组患儿治疗前后血气指数与呼吸功能、相关临床指标与并发症.结果 实验组患儿治疗后pH、PaO2与a/A水平显著高于对照组,PaCO2、FiO2、MAP、OI与肺动脉压显著低于对照组,差异有统计学意义(P<0.01);实验组患儿机械通气时间、NICU时间、NICU费用、总住院时间与总住院费用均显著低于对照组,差异有统计学意义(P<0.01);两组患儿总体并发症发生率(46% vs.44%)差异无统计学意义(P>0.05).结论 早期机械通气联合一氧化氮吸入治疗新生儿重度呼吸衰竭的疗效显著,且安全性较高,具有借鉴性.

Effect of early mechanical ventilation combined with nitric oxide inhalation in the treatment of severe respiratory failure in neonates
Abstract:Objective To investigate the curative effect of early mechanical ventilation combined with nitric oxide inhalation in the treatment of neonatal severe respiratory failure.Methods 100 neonates with severe respiratory failure hospitalized from June 2014 to June 2015 were randomly divided into two groups.Control group was treated with early mechanical ventilation,and experimental group was treated with nitric oxide inhalation on the basis.And compared blood index,respiratory function,related clinical indicators before and after treatment,and complications of two groups.Results The levels of pH,PaO2,and a/A after treatment in experimental group were significantly higher than those in control group,PaCO2,FiO2,MAP,OI,and pulmonary artery pressure were significantly lower than those in control group,with statistically significant differences (P<0.01).The duration of mechanical ventilation,NICU time,NICU costs,total hospitalization time,total hospital cost in experimental group were lower than those in control group,with statistically significant differences (P<0.01).There was no statistically significant difference in the overall complications rate between two groups (46% vs.44%;P>0.05).Conclusion Early mechanical ventilation combined with nitric oxide inhalation in the treatment of neonatal severe respiratory failure has significant effect and high safety,which is of reference.
