双负荷剂量氯吡格雷联合阿司匹林治疗脑梗死的疗效及安全性

目的 探究双负荷剂量氯吡格雷联合阿司匹林治疗脑梗死的效果及用药安全性.方法 选取2013年7月至2015年7月收治的90例脑梗死患者为观察对象,按照随机数字表法将患者分为标准剂量组和双负荷剂量组,每组45例.标准剂量组患者给予氯吡格雷75 mg/d+阿司匹林100 mg/d治疗,双负荷剂量组患者给予第1天口服氯吡格雷300 mg+阿司匹林300 mg,以后服用氯吡格雷75 mg/d+阿司匹林100 mg/d治疗.连续治疗8周,对比两组疗效及药物副作用.结果 双负荷剂量组患者疗效明显高于标准剂量组(97.78％ vs.84.44％;Z=7.717,P=0.042).两组患者药物不良反应发生率差异无统计学意义(P＞0.05).结论 双负荷剂量氯吡格雷联合阿司匹林治疗能够有效改善脑梗死患者临床症状,具有安全高效和副作用少等特点,值得在脑梗死治疗中推广应用.

Efficacy and safety of double loading dose of clopidogrel combined with aspirin in the treatment of cerebral infarction

Objective To explore the effect of double loading dose of clopidogrel combined with aspirin in the treatment of cerebral infarction and the safety of drug use.Methods 90 patients with cerebral infarction between July 2013 and July 2015 were selected as the observation object,divided into standard dose group and double loading dose group according to the random number table,45 cases in each group.Standard dose group was treated with clopidogrel 75 mg/d + aspirin 100 mg/d,while double loading dose group was given oral clopidogrel 300 mg + aspirin 300 mg on the 1st day,and then clopidogrel 75 mg/d + aspirin 100 mg/d,continuous treatment for 8 weeks.Compared curative effect and drug side effects of two groups.Results The curative effect in double loading dose group was better than that in standard dose group (97.78％ vs.84.44％;Z=7.717,P=-0.042).There was no statistically significant difference in the incidence of adverse drug reactions between two groups (P＞0.05).Conclusion Double loading dose of clopidogrel combined with aspirin can effectively improve the clinical symptoms of patients with cerebral infarction,with safety,high efficiency,and less side effects,it is worth to be applied in the treatment of cerebral infarction.
