尿α1酸性糖蛋白在预测T2DM患者糖尿病肾病进展中的价值

摘    要:目的 探讨尿α1酸性糖蛋白(AAG)预测2型糖尿病(T2DM)患者糖尿病肾病(DN)进展中的价值.方法 收集2008年7月至2011年10月入院治疗的T2DM患者100例,分别检测其尿AAG、随机尿微量白蛋白(MABL)和肌酐水平.将T2DM患者按尿AAG水平分为尿正常AAG组(NAAGU)组、微量尿AAG组(MAAGU)组和尿大量AAG组(MaAAGU)组,以MALB与尿肌酐的比值(ACR)为肾组织损害进展的参照标准,随访观察各组患者尿AAG水平与肾损害进展的关系.评价尿AAG水平对T2DM患者DN发生和发展的预测价值.结果 在NAAGU组、MAAGU组和MaAAGU组中,分别有7.14%、16.67%和37.5%的T2DM患者发生了DN的进展,有8.33%的MAAGU组患者发生了DN的缓解,而在MaAAGU组中无一例患者发生DN的缓解.结论 尿AAG测定可作为T2DM的辅助诊断指标之一,对T2DM患者DN的发生发展有较好的预测价值.

Clinical significance of urinary Alpha-1 acid glycoprotein in predicting the progression of diabetic nephropathy in type 2 diabetic patients

Abstract:Objective To investigate the value of urinary Alpha-1 acid glycoprotein (AAG) in predicting the progression of diabetic nephropathy(DN) in type 2 diabetic(T2DM) patients. Methods A total of 68 patients with T2DM were recruited from July 2008 to October 2011. The urinary AAG,random urinary Albumin (MA) and Creatinine (Cr) were measured. T2DM patients were further divided into 3 subgroups according to urinary AAG. Follow-up observation of the relationship between the urinary AAG and urinary albumin excretion rate (UAER) was carried in each group. Results Renal function aggravation occurred in 7.14%, 16.67% and 37.5% of patients with T2DM in NAAGU,MAAGU and MaAAGU group, respectively. 8.33% of patients with T2DM had DN remission in MAAGU group, but there was no DN remission occurred in patients of MaAAGU group. Conclusion Urinary AAG can be used as an assistant diagnostic index for T2DM and it is also helpful for predicting the development and progression of DN in T2DM patients.
