某院甲状腺手术围手术期预防用抗菌药物调查分析

目的调查某院非毒性甲状腺肿次全切除手术患者围术期预防性使用抗菌药物的情况，促进甲状腺手术合理用药，节约医疗费用。方法采用回顾性调查分析方法，选取某院2011-01-12非毒性甲状腺肿次全切除手术患者357例，依据《抗菌药物临床应用指导原则》、2011全国抗菌药物临床应用专项整治工作、围术期患者预防使用抗菌药物合理性评价标准等，分析其预防用药的合理性。结果该类患者平均住院日为（6．57±2．13）d，平均药物费用为（2236．06±872．19）元，抗菌药物使用率为20．72％，以7月份为分界线，前后抗菌药物使用差别较大，下半年情况明显改善。结论该院甲状腺手术围术期预防使用抗菌药物的不合理情况经过严格整治后明显减少，临床合理用药水平得到较大提高。

Analysis of Antibiotics Preventive Application during Perioperative Period of Thyroidectomy Surgery in A Hospital

Objective To investigate antibiotics preventive application during perioperative period of subtotal thyroidectomy surgery for nontoxic goiter, in order to promote the rational application of drugs, and to save health care costs. Methods A total of 357 cases of subtotal thyroidectomy surgery for nontoxic goiter in 2011 were retrospectively studied on the rationality of antibiotics preventive application, on the basis of Guidelines for Clinical Application of Antibacterials,2011 national rectification work of clinical antibacterials application, evaluation standard for rationality of antibiotics preventive application in patients during perioperative period, etc. Results The average hospitalization time was （6.57 4±2.13 ） d, and the average medicine cost was （2 236.06 ± 872.19 ） yuan. The usage rate of antibaeterials was 20.72% ,and the status of antimicrobial application before and after July was markedly different, the second half of the year was better. Conclusion The unreasonable preventive application of antimicrobials during perioperative period of subtotal thyroidectomy surgery is significantly reduced after rectification work, and the level of clinical rational drug use is improved greatly.
