28例胰腺损伤的外科治疗及预后因素分析

目的：探讨胰腺损伤的外科治疗及影响预后的因素。方法：对我院1995年～2004年收治的28例胰腺损伤病例的临床资料进行回顾性分析。结果：本组28例中，术前确诊4例（14．3％），23例治愈（82．1％），5例死亡（17．9％），发生并发症13例（46．4％），术后胰漏是主要并发症。结论：胰腺损伤术前诊断困难，剖腹探查术是早期诊断最可靠的方法，根据术中探查情况采取简单而合理的手术方式以及正确的围手术期处理是影响胰腺损伤预后的主要因素。

Analysis of prognostic factors and surgical treament in 28 patients with pancreatic trauma

Objective:To investigate the prognostic factors and surgical treament of the pancreatic trauma.Methods:The clinical data of pancreatic trauma from 1995 to 2004 were collected and retrospectively analysed.Results:In this series,four cases(14.3%)got preoperative diagnosis,23 cases(82.1%)were cured,and 5 cases died(17.9%),complications occurred in 13 cases(46.4%).Pancreatic leak was the main complications.Conclusion:Preoperative diagnosis of the pancreatic trauma is difficulty.Exploratory laparotomy is the most reliable way.According to operative exploration using simple and reasonable surgical procedure and perioperative management are the major prognostic factors for pancreatic trauma.
