氯丙嗪、氯氮平、利培酮对血糖和血脂的影响

目的　比较氯丙嗪、氯氮平、利培酮对血糖、总胆固醇、甘油三脂、高密度脂蛋白、低密度脂蛋白的影响。方法　对2 0 0 2年 1月～ 2 0 0 3年 12月我院住院的 12 0例单一服用氯丙嗪、氯氮平、利培酮治疗的精神分裂症患者 (各 4 0例 )治疗前和治疗 6w末的血糖、血脂四项进行组内统计分析。结果　氯丙嗪、氯氮平、利培酮对血糖和血脂四项均有影响 ,其中氯丙嗪组患者的血糖升高较治疗前有显著性差异 (P <0 .0 5 ) ;氯氮平组患者的血糖升高较治疗前有极显著性差异 (P <0 .0 1) ,总胆固醇、高密度脂蛋白变化较治疗前有显著性差异 (P <0 .0 5 ) ,且女性变化比男性显著 (P <0 .0 5 ) ;利培酮组对血糖、血脂的影响较小。结论　抗精神病药物对精神分裂症患者的血糖、血脂等有明显的影响 ,建议治疗前和治疗过程中应及时监测血糖和血脂的变化 ,防止并发症的发生

Effects on serum glucose and lipid in schizophrenics treated with chlorpromazine,clozapine or risperidone

Objective To study the effect on serum glucose and lipid in schizophrenia patients treated with chlorpromazine, clozapine or risperidone. Methods Received the records of schizophrenia inpatients who had been only using chlorpromazine or clozapine or risperidone in 2002-2003, analyzed the serum glucose and lipid before and at the end of 6 week after using drugs. Results All of chlorpromazine, clozapine and risperidone effected on the serum glucose and lipid. Glucose levels were significantly higher in chlorpromazine and clozapine teams, the TCHOL and TG and LDL-C and HDL-C levels changed significantly after using clozapine, TG and LDL-C levels were significantly higher in female than in male(p<0.05). Conclusion Anti-psychiatric drugs effected on serum glucose and lipid in schizophrenics. It is necessary to take preventive measures and test the serum glucose and lipid in schizophrenia patients treated with chlorpromazine, clozapine or risperidone.
