264株临床分离铜绿假单胞菌耐药性分析

摘    要:目的:了解医院铜绿假单胞菌的分布及耐药状况。方法:标本经分离培养,采用美国BD公司BBLCrystal细菌鉴定分析仪进行鉴定。结果:铜绿假单胞菌医院感染主要发生在呼吸科病房(37.1%),从临床标本中分离的264株铜绿假单胞菌中196株来源于痰,对氨苄西林、环丙沙星的耐药率分别为100.0%、40.9%;对其他β-内酰胺类抗菌药物中的阿米卡星、哌拉西林、哌拉西/他唑巴坦耐药率分别为20.1%、54.1%、28.4%;亚胺培南耐药率达20.0%,头孢他啶的耐药率最低,为12.5%。结论:铜绿假单胞菌是下呼吸道感染的主要致病菌之一,其耐药机制复杂,有多重耐药的特性,希望临床医生在治疗铜绿假单胞菌的感染过程中充分考虑其耐药机制,选用耐药率低的药物,避免诱导铜绿假单胞菌对抗菌药物广泛耐药。

Abstract:Objective:To investigate the clinical distribution and drug resistance of Pseudomonas aeruginosa.Methods:All specimens isolated and cultured from patients in our hospital were identified by using the microorganism analyzer BBL Crystal, BD made in America.Results:The main departments in which frequetly Paeruginosa infection accurred was Respiration Departments(37.1%).From 264 strains of Paeruginosa 196 strains were isolated of sputum.The resistance rates to the third generation of AMP and CIP was 100.0% and 40.9% , respectively.The resistance rate to the other β-lactamases antibiotics such as AMK was 20.1%, PIP 54.1%, TZP 28.4%, the ratio of resistance for IPM was 20.0%, the lowest one was CAZ, 12.5%.Conclusion:Paeruginosa is one of the main bacteria in the lower respiratory tract infection.The drug-resistant mechanism of Paeruginosa is very complex, including multidrug resistance characteristics.To avoid being produced β-lactamases and result in resisting drug widely , the antibiotics should be selected according to low drug-resistant rate and taking into account sufficiently its drug resistance mechanism in the treatment of Paeruginosa infection.
