彩色多普勒超声诊断飞行员左冠状动脉-肺动脉瘘一例并文献复习

目的 探讨冠状动脉瘘的影像特征、鉴别诊断及彩色多普勒超声在飞行员改装体检中的医学鉴定价值.方法 分析1例飞行员左冠状动脉-肺动脉瘘超声图像特征并复习相关文献.探讨应用彩色多普勒超声检查在飞行员改装体检中的必要性.结果 彩色多普勒超声显示心底部心肌间走行迂曲的异常血流束,经主肺动脉外侧壁注入肺动脉内.超声诊断:左冠状动脉-肺动脉瘘.飞行结论:改装并原机种飞行不合格结论彩色多普勒超声检查能清晰显示冠状动脉瘘,并能追踪观察其走行、部位、内径和大小,超声诊断与冠状动脉造影结果相符.冠状动脉瘘早期无特异性临床表现,彩色多普勒超声心动图诊断冠状动脉瘘直接、安全、快捷,具有重要的临床价值,是明确诊断的首选方法.

A case of color Doppler ultrasound diagnosis of pilot's left coronary-pulmonary artery fistula and literature review

Objective To explore the characteristics and diagnosis of coronary artery fistula images,and evaluate the value of color Doppler uhrasound examination for the transition pilot in medical assessment. Methods The characteristics of a pilot'S ultrasound images of left coronary artery pulmonary artery fistula were analyzed and related literature was reviewed.Investigation also conducted the necessity of applying color Doppler uhrasonography in transition pilot'S physical examinatiom Results The imagine of color Doppler ultrasound showed that the abnormal blood flow beam was tortuous between myocardia of cardiac base,and perfused into pulmonary artery via lateral wall of the main pulmonary artery.The left coronary artery-pulmonary artery fistula was diagnosed by ultrasound diagnosis.The pilot was disqualified both for flying original and transition type of aircraft. Conclusions Color Doppler uhrasonography can clearly show coronary artery fistula.include its running track,location,diameter and size.Those are consistent with the results of coronary angiography.Color Doppler echocardiography shows its clinical value in diagnosing coronary arterv fistula with a direct,safe and facile way since no specific clinical manifestations visualized at early stage.It's recommended as the first choice in clinical examination of coronary artery fistula
