胆道感染病原菌谱及其耐药性

摘    要:目的 探讨胆道感染病原菌谱及耐药性。方法 胆道感染住院手术患者的胆汁行细菌培养,K-B法或两倍稀释法行药敏测定。结果 胆汁标本中分离出病原菌614株,培养阳性率达90.4%,其中革兰阴性细菌460株、占74.9%,革兰阳性细菌133株、占21.7%;药敏结果提示铜绿假单胞菌、肠球菌属、葡萄球菌属出现多重交叉耐药,其中革兰阴性杆菌对亚胺培南、第三代头孢菌素及环丙沙星较为敏感,革兰阳性球菌对万古霉素和亚胺培南敏感。结论 胆道感染的主要致病菌是大肠埃希菌、克雷伯菌属、肠球菌属、肠杆菌属、变形菌属及假单胞菌属和葡萄球菌属,出现多重耐药。

Abstract:OBJECTIVE To realize the bacterial spectrum and drug resistance in inpatients with biliary infection. METHODS Biliary specimens from inpatients with biliary infection were cultured for bacterial identification, and the antimicrobial activity tests were determined by K B or agar dilution methods. RESULTS A total of 614 strains, 74.6% of which were Gram negative bacilli and 21.7% of which were Gram positive ones. Almost of the strains showed resistance to all the agents tested in different levels, also the multidrug resistance emerged. Imipenem, ceftazidime and ciprofloxacin are the relatively active against the Gram negative bacilli. CONCLUSIONS The main pathogenic microbes of biliary infection were Escherichia coli, Klebsiella pneumoniae, Enterococcus, Enterobacter, some were with multidrug resistance in high levels.
