2型糖尿病颈动脉粥样硬化患者血清二肽基肽酶4和氧化低密度脂蛋白水平改变

摘    要:目的探讨T2DM合并颈动脉粥样硬化(CAS)患者血清二肽基肽酶4(DPP-4)和氧化低密度脂蛋白(ox-LDL)水平改变。方法回顾性分析2020年1~10月于宜昌市第二人民医院内分泌科住院的T2DM患者120例,根据颈动脉彩超检查颈动脉内膜中层厚度及直接观察有无动脉粥样斑块形成,分为单纯T2DM组(T2DM,n=60)及T2DM合并CAS组(CAS,n=60),同期选取性别、年龄匹配的本院体检健康者30名为正常对照(NC)组。分析各组血清DPP-4、ox-LDL指标差异。结果CAS、T2DM组血清DPP4、ox-LDL水平高于NC组(P<0.05或P<0.01)。CAS组血清DPP-4、ox-LDL水平高于T2DM组(P<0.05或P<0.01)。Spearman相关分析显示,T2DM患者血清DPP-4水平与ox-LDL、HbA1c、HOMA-IR、CIMT呈正相关(P<0.05或P<0.01),与HDL-C呈负相关(P<0.05)。Logistic回归分析结果显示,DPP-4、ox-LDL是T2DM合并CAS的危险因素。结论T2DM合并CAS患者血清DPP-4、ox-LDL水平升高,是T2DM合并CAS的危险因素。

Changes of serum dipeptidyl peptidase-4 and oxidized low-density lipoprotein levels in type 2 diabetic patients with carotid atherosclerosis

Abstract:Objective To explore the changes of serum dipeptidyl peptidase-4(DPP-4)and oxidized low-density lipoprotein(ox-LDL)levels in patients with T2DM combined with carotid atherosclerosis(CAS).Methods A retrospective analysis of 120 T2DM patients in the Department of Endocrinology,Second People’s Hospital of Yichang city from January 2020 to October 2020 was adopted.All the included T2DM patients underwent carotid artery color Doppler ultrasound examination,and carotid artery intima was obtained by carotid ultrasound.According to the intermediate thickness(CIMT)and direct observation of atherosclerotic plaque formation,the participants were divided into T2DM group(n=60)and CAS group(n=60).At the same time,30 healthy people undergoing physical examination were selectedas controls matched by gender and age,named as NC group(n=30).The differences of serum DPP-4 and ox-LDL indexes among different groups were analyzed.Results The levels ofserum DPP4 and ox-LDL in CAS group and T2DM group were higher than those in NC group(P<0.05 or P<0.01).The serum DPP-4 and ox-LDL levels in CAS group were higher than those in T2DM group(P<0.05 or P<0.01).Spearman correlation analysis showed that the serum DPP-4 level of T2DM patients was positively correlated with ox-LDL,HbA1c,HOMA-IR,and CIMT(P<0.05 or P<0.01),and negatively correlated with HDL-C(P<0.05).Logistic regression analysis showed that serum DPP-4 and ox-LDL levels were independent riskfactors of T2DM combined with CAS.Conclusion Serum DPP-4 and ox-LDL levels in T2DM patients with CAS are elevated,which may be independent risk factors affecting the vascular complications of T2DM.
