左乙拉西坦用于儿童癫痫的临床效果观察

目的探讨左乙拉西坦治疗各种类型儿童癫痫的临床效果。方法选择新疆维吾尔自治区人民医院神经内科2011年1-12月治疗的儿童癫痫共86例，按照随机平行设计研究的方法，将其分为观察组43例和对照组43例。观察组使用左乙拉西坦口服治疗，根据患儿个体情况调节剂量。对照组使用丙戊酸治疗，共治疗12周。经过门诊复诊以及电话随访的方式，对临床疗效进行判定。结果两组患儿均未失访，观察组的癫痫控制人数为30例，显效为8例，观察组和对照组患者的临床治疗效果差异有统计学意义。观察组的癫痫发作频率为（2．3±1．0）次/个月，脑电图异常率为20．9％，观察组患儿的癫痫发作频率显著低于对照组（P〈0．05），脑电图异常率差异无统计学意义（P〉0．05）。观察组治疗后发生不良反应15例，主要表现为嗜睡4例，胃肠道不适3例，头晕1例，情绪异常7例，未影响治疗。两组患儿的不良反应发生率差异无统计学意义（P〉0．05）。两组患儿均完成整个治疗过程。结论左乙拉西坦用于儿童癫痫的治疗效果确切．不良反应较轻。是一种值得推荐的儿童安全抗癫痫药物。

Observation on clinical effect of Levetiracetam used in children with epilepsy

Objective To investigate the clinical effect of Levetiracetam for various types of epilepsy in children~ Methods 86 cases of epilepsy children from January to December 2011 treated in Neurology Department of Xinjiang Uighur Autonomous Region People＇s Hospital were selected. According to the method of random parallel design study, they were randomly divided into observation group of 43 cases and control group of 43 cases, who all signed informed consent. The Observation group used Levetiracetam oral treatment according to the individual situation of adjusting dosage. The control group was used Valproate treatment with 12 weeks. After return visit and telephone follow-up, the clinical efficacy was evaluated. Results The two groups of children were not lost to follow-up, 30 cases of observation group were controlled, 8 cases were excellent, the clini- cal therapeutic effect of observation group and control group had statistically significant differences. The seizure frequency of observation group was （2.3±1.0） times per month, and the abnormal rate of EEG was 20.9%. The seizure frequency in children of observation group was significantly lower than that of the control group （P 〈 0.05）, the abnormal rate of EEG showed no significant difference （P 〉 0.05）. 15 cases had adverse reactions of observation group after treatment, which mainly contained 4 cases of lethargy, 3 cases of gastrointestinal discomfort, 1 case of dizziness, 7 cases of abnormal emotion, but these reactions did not affect the treatment. Adverse reactions of the two groups had no significant difference （P 〉 0.05）. Two groups of children completed the whole treatment process. Conclusion The curative effect of Levetiracetam for the treatment of epilepsy in children is effective, with less adverse reactions, and it can be recommended as a child safety of antiepileptic drug.
